How to request a shoulder patch?
When patches are available are used and have been taken off of worn out uniform shirts. Availability is limited and we do not always have patches available. The cost is $10 per patch.

- Mail your request in writing with payment.
- Request must have name and return address in the United States.
- Patches will not be sent outside the United States.
- Payment of $10 in the form of a personal check or cashiers check to the City of Mapleton.
- A patch and receipt will be mailed to you.

We receive many personal requests for our patches at no cost but unfortunately we do not have the staff time or financial resources to fulfill all the free patch requests.
LAW ENFORCEMENT AGENCIES
We will trade one of our used patches (as they are available) for one of your patches (used or new). Your request must be on a department letterhead, mailed via USPS and include a patch from your agency. We will only send our patch directly to another law enforcement agency. This is only for law enforcement agencies in the United States.
